If you have BCBS/Anthem PPO, Aetna PPO, or United Healthcare/UMR PPO, I’m in-network with those plans. Use the Insurance Coverage Checklist below to guide your conversation with the insurance representative. It includes the exact questions to ask.
*I accept insurance for time spent in consultations. My Concierge-Insurance Hybrid Package Fee is to cover the time I spend working on your care outside of consultations, as well as services that are typically not covered by insurance like HIPAA secure texting.
If you have an out-of-network plan or your insurance doesn't cover nutrition services, no worries, I also offer a cash-pay program as an alternative.
